Answer from PSC Doctor:
John Griffin, MD
jgriffin responded:
I have not yet had an infection from fat transfer to the breast to my knowledge. Regarding size, with use of external tissue expansion first, we try to predictably achieve somewhere between 200 to 300 cc of long term volume increase.
Answer from PSC Doctor:
Bruce W. Van Natta, MD
bvannatta responded:
As Dr. Griffin mentioned, I too have had no infections but clearly that is always a possibility. We do give a single IV dose of antibiotic prior to the procedure. The “take” of fat grafts is usually around 50% or so of the volume we place. I tell most of my patients that they will likely need at least one additional surgical procedure to achieve a modest augmentation. We now have the ability to bank fat from the first surgery so that a second procedure involves minimal downtime and recovery. Please be aware that depending on your individual variables and especially if a second surgery is required the costs for lipoaugmentation can be considerably higher than those for a standard implant augmentation.
Answer from PSC Doctor:
William P. Adams, Jr., MD
CMO responded:
Also keep in mind fat injection to the breast for augmentation is an unproven entity at this point and should be done under study protocols ; Unlike breast reconstuction revision where fat has been used reliably and has good data. Based on results at several live surgery meeting the past year – breast augmentation with fat appears to be able ot produce 100-200cc enlargement after a single injection. This volume tends to be in the lower pole of the breast. The safety aspects of this technique is still being investigated.
Lastly, the patient experience of fat breast augmentation compared to an implant breast augmentaiton is vastly different with the implant augmentation having superior recovery including being able to go shopping the day of surgery, predictable results, and proven excellent outcomes. rgrant responded:
I agree completely with Dr. Adams’ comments. In my opinion when you compare implants vs fat transfer techniques -as they exist today -on cost, predictability of results, safety, infection rates, recovery, need for additional subsequent procedures, and documentation of long term results implant based augmentation remains the ‘gold standard’ .
